Thanks so much for your willingness to organize a new chapter in your area. I'm excited to finally be able to launch Birthday Project Chapters, and need you to answer just a few questions. Feel free to offer as much or as little information as you want and remember that I am happy to help assist you in whatever ways I can to get your Chapter up and running.

Once I have your answers, I'll post the full list of where all the groups meet (on the website and Facebook Page). I'll also provide public contact information, for the Chapters who choose to provide it.
